Physical Examination
Students registering for Athletics must have a current physical examination and a completed Health History questionnaire on file in the nurse's office before the student is authorized to participate in practice/tryouts. Physicals are valid for *12 months* from the date of the last exam.
Step 1: Submit Health Forms
All Health Forms must be submitted as a hard copy to the school nurse.

If you have Asthma, or a life threatening allergy you will need to update the MD orders for the 2018-19 school year. Students will not be eligible to practice without current MD orders on file. Students can drop off completed Asthma and allergy forms to the Health Office or they may be faxed to 609-465-4504. **Filled out 1 time per year**
